Donna Zazworsky, MS, RN, CCM, FAAN, is an internationally known expert in case management and disease management with community populations. She is managing partner for Case Manager Solutions, LLC, and director of Grants, Partnerships and Policy for St. Elizabeth of Hungary Community Health Center---a large clinic that serves more than 18,000 uninsured in Tucson and the surrounding communities.
